Registration

All uninsured individuals who would like access to the services offered by Fort Bend Family Health Center, Inc. must go through registration in order to determine eligibility for reduced-fee programs. If an individual is eligible for Medicaid, they will be referred to our on-site Medicaid worker. Once registered with FBFHC,  a patient will be certified for one year.  For a list of documents needed to register, please click on the links below.
